Thyroid management for a body running on empty

THYROID TESTING BEYOND TRADITIONAL "NORMAL" LABS

What does the thyroid do?

Your thyroid is a small gland at the base of your neck that regulates nearly everything your body does at rest: your metabolism, your heart rate, your body temperature, your digestion, your mood, and your ability to think clearly. When it underproduces, nothing works quite right, and the symptoms tend to build so gradually that many people chalk them up to stress, aging, or just not taking good enough care of themselves.

Hypothyroidism, or an underactive thyroid, is one of the most commonly undertreated hormonal conditions, particularly in women. It's also one of the most frequently missed because standard lab panels often measure only TSH, which doesn't give a complete picture of what your thyroid hormones are actually doing.

Signs your thyroid might be causing issues

Thyroid symptoms are easy to dismiss individually. It's the pattern that points to something worth investigating

Why thyroid issues are so often missed

The most common reason patients arrive here after years of feeling unwell is that a previous provider checked TSH and called it normal. TSH is a useful first marker, but it's an indirect measure, it tells you how hard the pituitary is working to stimulate the thyroid, not what the thyroid is actually producing or how well your body is using it.

A complete thyroid picture requires looking at free T4, free T3, reverse T3, and often thyroid antibodies. Free T3 is the active form of thyroid hormone your cells actually use. Some patients produce adequate T4 but convert it to reverse T3, an inactive form, instead of active T3, which means standard treatment with T4 alone won't resolve their symptoms even when labs look acceptable. Thyroid antibodies reveal whether an autoimmune process like Hashimoto's thyroiditis is driving the problem.

What does your thyroid management appointment look like?

It starts with a comprehensive thyroid panel, not just TSH, alongside a thorough review of your symptoms and history, because lab results and how you feel don't always point in the same direction and both matter. From there, we'll talk through what the results actually mean and what treatment, if any, is clinically appropriate.

Thyroid management is ongoing: dosing is adjusted based on how you respond and how your labs shift over time, not set once and left. For patients where thyroid dysfunction overlaps with other hormonal concerns, which is common, particularly around perimenopause and menopause, we can look at hormone therapy and thyroid management together rather than treating them in isolation.

What life can look like on the other side of your thyroid management journey

Things that have felt impossible for months or years stop requiring so much effort.

Balanced energy

When levels normalize, so does your actual cellular energy output.

The scale shifts

Restoring thyroid function brings that baseline back up, and weight typically starts to follow.

Sharpened cognition

Most patients notice brain fog lifting within weeks of reaching optimal levels, often before other symptoms fully resolve.

Body warmth generates on its own again

Cold intolerance is one of the earliest symptoms and one of the most satisfying to watch resolve.

Stronger hair, nails & skin

Why patients choose Collins Vitality for thyroid management

A lot of patients who come to us for thyroid evaluation have already been told their levels are fine by someone who only ran TSH. We run the full panel because that's the only way to actually know what's happening. And we don't separate thyroid health from the rest of the picture: if hormones, weight, and thyroid are all contributing to how you feel, we can look at all of them together rather than sending you to three different providers. We see patients from Glendale and across the greater Phoenix area for exactly that reason.

Common questions about thyroid management

  • Can thyroid problems cause brain fog?

Yes, and it's one of the most consistent complaints. Causes of brain fog include several conditions, but hypothyroidism is among the most common and most fixable. Thyroid hormone directly affects neurological function, including processing speed, memory consolidation, and mental clarity. When T3 levels are low, the brain simply doesn't work as efficiently, and most patients notice cognitive improvement as levels normalize.

  • Why am I always feeling cold?

Cold intolerance is a classic hypothyroid symptom. Thyroid hormone regulates thermogenesis, your body's ability to generate and maintain heat. When production is low, core temperature regulation suffers, which is why feeling cold all the time, even in warm environments or when everyone around you is comfortable, is one of the more diagnostically specific symptoms. It's also one of the first things that tends to improve with appropriate treatment.

  • Can an underactive thyroid cause weight gain?

Yes. Thyroid hormone governs your basal metabolic rate, the number of calories your body burns at rest. When it's low, metabolism slows, and weight tends to accumulate even without changes in diet or activity. This is also why weight loss is so difficult for undiagnosed or undertreated hypothyroid patients regardless of effort. Addressing thyroid function is often a prerequisite for meaningful progress on weight.

  • Is thyroid dysfunction related to menopause?

Frequently, yes. Estrogen affects the proteins that transport thyroid hormones in the blood, which means hormonal changes during perimenopause and menopause can directly influence thyroid function and how symptoms present. Many women are diagnosed with thyroid conditions during this transition, and both issues often need to be addressed together. Our women's hormone therapy and thyroid management can be coordinated as part of the same care plan.

  • What if I've already been told my thyroid is fine?

If only TSH was tested, you may not have the full picture. TSH is one data point, and a useful one, but it doesn't tell you whether your T4 is converting to active T3 efficiently, whether reverse T3 is blocking thyroid hormone from being used, or whether thyroid antibodies suggest an autoimmune process. A complete panel often reveals what a single-marker test misses.
